Welcome to the Chronic Disease Management (CDM) Toolkit
The CDM Toolkit is a resource available for Yukon physicians, community nurse practitioners and members of their care teams, e.g. nurses, medical office assistants, clerks, pharmacists, dietitians, specialists. It is a tool to help with the process of providing clinical guideline-directed care to patients with chronic conditions.
